Prior to the creation of the Department of Homeland Security in 2002, U.S. Customs belonged to the Treasury Department. After the reorganization in 2003, U.S. Customs was transferred to DHS and renamed the Bureau of Customs and Border Protection (CBP). All previous Customs Forms (CF) have been revised to replace the “CF” with “CBP”. The form number and the date of the last revision are usually shown at the bottom right corner of the form.

For Individual Serially Numbered Forms
Serially numbered forms are not available electronically (e.g., CF 5129, Crew Member's Declaration). Please contact the Distribution Center via fax on 317-290-3046 to obtain copies of individual forms in bulk. Local Port Offices should have small quantities on hand for those who require only a few copies.

For Blocks of Serially Numbered Forms
On company letterhead, fax your request for a block of serial numbers to the Forms Manager on 202-927-1450.

Printing
Forms printed in inks other than black may now be printed in black. Original forms with multiple components printed on color stock, must also be printed on color stock. Procedures for submitting forms has not changed.

Format
Forms are available in three formats. Print only produces an Acrobat PDF document that can be printed and completed off-line. Online allows electronic completion and printing of the form. Wizard divides longer forms into panels or sections that can be completed one by one and then printed as one document. Final output for all formats is Acrobat PDF.
